The jawline pattern
Hormonal acne is not a distinct lesion type — it is a pattern. Deep, tender bumps arrive along the sides of the jaw, chin and neck. They tend to be cyclical, worsening in the week or so before a period, and they persist beyond the mid-twenties when most non-hormonal acne has settled. Post-inflammatory pigmentation is almost universal in Indian skin because the lesions are deep and inflamed.
Why hormones drive acne
Androgens — testosterone and its more potent form DHT — bind receptors on the sebaceous gland and instruct it to make more oil. More oil means more clogged pores, more C. acnes multiplication and more inflammation. Anything that shifts the androgen-to-oestrogen ratio can therefore trigger a flare: puberty, the late-luteal phase of the menstrual cycle, stopping combined oral contraceptives, PCOS, perimenopause, high stress (cortisol pushes ovarian and adrenal androgens up), and some medications.
PCOS and adult hormonal acne
PCOS is the most common structural driver of adult hormonal acne we see in Mumbai women. Insulin resistance is central — insulin drives the ovaries to make androgens, which drive sebum, which drives acne. It also explains why weight, cycles, hair and acne so often move together in PCOS.
This is why our hormonal acne plan almost always integrates PCOS nutrition. Treating the skin without addressing the metabolic driver leaves half the job undone.
Tests we may recommend
Testing is decided at consultation, not by default. A typical panel when we do test:
- LH, FSH (day 2–5 of cycle)
- Testosterone — total and free
- DHEAS
- Prolactin
- TSH — thyroid drives skin quality and hair
- Fasting insulin and HbA1c — the insulin-resistance markers
- Vitamin D and B12 — commonly deficient in Indian women, both affect skin
Topical plan for hormonal acne
The routine is barrier-first, retinoid-anchored, pigment-aware.
- Gentle amino-acid or salicylic cleanser, twice a day.
- Nightly retinoid (adapalene or a well-formulated retinal) — the single most impactful topical for hormonal acne.
- Azelaic acid — pairs beautifully with retinoids for inflammation and pigment.
- Niacinamide for oil balance and barrier support.
- Mineral or hybrid SPF 50, reapplied — pigment marks will not fade without this.
Systemic and hormonal options
When topicals alone are not enough, systemic options come into play. These are prescription decisions made together with your treating doctor.

- Protein at every meal to steady insulin

- Strength training 2–3x weekly (best insulin-sensitising exercise)
- Sugary drinks and packaged juices
- Whey isolate supplements during flares
- Skim milk in large quantities
- Chronic under-eating and long fasting windows
- Skipping breakfast when hormonal acne is active

How do I know if my acne is hormonal?+
Classic signs: painful deep breakouts along the jaw, chin and neck; a cyclic pattern in the 7–10 days before your period; worsening after 25; irregular cycles; unwanted facial hair or hair thinning at the crown. If several of these fit, hormonal drivers are likely.
Do I need blood tests for hormonal acne?+
Not always. Testing is recommended when there are other features suggestive of PCOS (irregular cycles, weight change, hirsutism, hair thinning) or when acne is not responding to standard care. Typical panel: LH/FSH, testosterone (total & free), DHEAS, prolactin, TSH, fasting insulin and HbA1c.
Can hormonal acne be treated without oral contraceptives?+
Yes. Many of our patients clear beautifully with a combination of medical-grade topicals, in-clinic peels or LED, and PCOS-focused nutrition. Oral therapy is reserved for cases that need it — it is not the first step.
Will fixing PCOS clear my acne?+
In most cases, addressing insulin resistance and inflammation reduces the intensity and frequency of hormonal flares within 3 months. It rarely clears acne single-handedly — you still need a topical and clinic plan.
Is spironolactone safe for hormonal acne?+
Spironolactone is an androgen-blocking option that can be very effective for adult female hormonal acne. It is a prescription decision made with your treating doctor and is not suitable for pregnancy.
Why does my acne flare before my period?+
In the luteal phase, progesterone rises and the ratio of oestrogen to androgens shifts. Skin becomes oilier and pores more reactive. Consistent skincare, avoiding new products in this window and pre-emptive salicylic use can flatten the peak.
